First, is there a link to a list of how many points each repuation level is?
Are there any kind of hardcoded reputation gains or losses included in the game or is (hopefully) all trigger?
What I want to do is to use reputation as a measure of success instead of a measure of karma, and put a check in every mission to end the game if your overall performance is too low.
That way you can have it where the game doesn't end immediately if you let a hostage die or fail a mission. If you keep screwing up, you eventually get kicked out, but if you go out on the next couple of missions and perform heroicly you can redeem yourself. Or instead of ending the game immediatly, it could open up some sort of suicide mission you have to take to get back in their good graces.

The reputation levels are IIRC,
Savior of the Wasteland, 1001 reputation points
Guardian of the Wastes, 601 reputation points
Shield of Hope, 301 reputation points
Defender, 101 reputation points
Warrior, 0 reputation points
Betrayer, - 100 reputation points
Sword of Despair, - 300 reputation points
Scourge of the Wastes, - 600 reputation points
Demon Spawn, - 1000 reputation points
Or something to that effect, the reputation is all handled by triggers. If you're feeling really ambitious instead of ending the campaign if your rep gets too low, how about opening up an alternate path where you turn into a raider/slaver?

Except that bit about attacking neutral/friendly people, which causes a hard-coded Rep loss. It's 100 pts./person, isn't it? (This can be minimized by having an Index x-fer to a hostile team if the player kills one of them, though. Under the right circumstances this wouldn't be as hard to do as it sounds.
