I'd prefer not only bad mutations but positive mutations like a third eye that would improve perception, or a third item slot from an extra arm.

I'd take the realistic damage option, so that radiation is finally something to fear, not like in Fo2. Perhaps it could fuck up your skin and make you ugly and that would modify npc reaction of course. The 3rd arm ideas seem cool now, but i sure wouldn't like to see them in real Fo3.
