a double barrel shot gun made from two car exaust pipes that fires
home made rounds made up of broken glass and nails and various other items.
a punch shot gun would kick ass.
a hand held bear trap glove, one hit and whatevet you hit gets a big
chunk ripped out.

Real life guns don't fit in Fallout. So it'd be better to invent a totally new grenade launcher rather than adding an m79 shite.
And the Fo2 devs must have been morons, because with the source code to Fo's engine, adding a grenade launcher would not be a major task.

The sawed off shotgun should be the default gun you start off with and find ammo for everywhere.
Starting weapons:
Sawed-off shotguns
Samurai sword
Tommy-gun
Morningstar
