The frustrating thing is that Bethesda are in a position to innovate. Even if FO3 fails commercially, they have enough funds to support that - the more likely option is, even if they make FO3 as hardcore and as niche as possible, it'll sell loads because of Bethesda's name. That's a great opportunity...
Same with Bioware, actually. Which makes it all the more disheartening that they tend to stick to the same formula with each game. Do people buy bioware games these days because of the game itself, or because of the company making them?
