My guess is that the downside is the same that is facing old people already: we know how to keep the body functioning well past its due date but don't understand the mind at all so the mind just melts.
Interesting though, I guess in the near future it's literally kill or be killed, otherwise you'll just go on forever. Kind of a neat dilemma between humanism and capitalism too: should we support and care for people who have Alzheimer's or whatever and will be an economical burden to the society for who knows how long, or will it be all Logan's Run and firing squads

I wonder how it affects the consciousness if you undergo brain rejuvination. Fun trade off if you lost memories. Life extention for pets is good.
