Who's the Follower spy and how can I get him?

It's a bug. It's one of the quests that weren't finished in FO1. There's no way to solve it.

There is a Follower spy in the cathedral. Look for Laura in one of the small rooms near the entrance.

Another quick question: When I asked Nicole aabout the Followers, she said the hisotry was in the last bookcase top shelf, has anyone ever found it?

I've never looked, and I wouldn't bother. It's very unlikely that such a book actually exists.
