I appreciate the feedback on my other two questions (gets me to thinking a lot about stuff). So here is another :)

I think to advance one or possibly two levels per mission seems adequate.

Also depends though, the size of map and what short of enemies quests you're getting(and in campaigns, the difficulty level of the next map).

Hmm.. this is interesting. I think I have established that most FOT players prefer a tough challenge and/or strategic situation over tweaking characters and/or leveling up?
But I will assume that both of these are always preferable to only one or the other. And I now know that I should put a little more focus on strategy than tweaking and other stuff.
Now I don't have to worry about how hard I make my maps 8)

In the case of a campaign I'd say that -- generally speaking -- one level gained/mission is fine, although in some circumstances you might want to go for two levels/mission. For instance, you could give the player the opportunity to gain two levels in a single mission in the concluding mission of a chapter where the player is busy destroying the hub of a faction.
Also, you might want to consider putting some things in your missions that are purely optional. This optional area would be much harder than the required portion, but could yield a second level, more and better equipment and/or someting like access to a mission which the player would not ordinarily receive.

The campaign im working on right now pretty much lets the player go where they want and do what they want after about the 2nd mission. The only reason not before is so that they can have a few characters before running off to the wild wazoombaville yonder and back.
Im trying for that good old Fallouty feel as much as possible. Maximum freedom and good old fashion fun :)
Heck, im even going so far as to have multi-map areas. Although I have not experimented with that yet, I plan to do so very soon. As soon as I have my first 4 maps mostly done anyway.
The only problem might be my story and the humor. Im sure not everyone will get it or appreciate it. But hey, its my story so that's that :)
