He's like some kind of development troll. It looks like he has food in his cheeks. Or possibly the torn off genitals of underage boys.
It's like they found him under a bridge and felt he had wasteland experiecne so they hired him for fallout 1 and 2, but then he got out for a few years and the guys at bethesda were not able to keep him under control, so he invented micronukes.
